EXPLANATION OF FORMS:
- FILL IT UP –
This is for Scouts that had 30 or more customers in the popcorn fundraiser.
- POPCORN POPPER – This is for Scouts that had 5-29 customers in the popcorn fundraiser.
- GROUP ORDER FORM – This is for Unit Leaders that wish to order a group of tickets (you can submit this form instead
of sending all of the forms from your Scouts).
- QFAD FAQs – This form contains general information about the Q For a Day event.
- LIABILITY WAIVER – All attendees at the Q For a Day must submit a signed liability
waiver.
IMPORTANT INFORMATION:
- The DUE DATE to submit your order is FRIDAY,
JANUARY 15TH (some
of the order forms were mistakenly labeled with “Wednesday, Jan. 15th)
- Processing Fee: The fee is $5 for each order, NOT for each ticket.
(there was a printing error and some of the order forms did not
list the $5 fee- we apologize for the inconvenience)
